What is Tesamorelin?
Tesamorelin is a longer-lasting version of growth hormone releasing hormone, the signal the brain uses to tell the pituitary gland to release growth hormone. It is the same active ingredient as Egrifta, the prescription injection the FDA approved in 2010 to reduce excess abdominal fat in adults with HIV-associated lipodystrophy, and it has the largest trial record of any peptide on this site. Here it is prescribed as a take-home vial you inject under the skin, after a free consultation and a review of your history.
Tesamorelin benefits and uses.
-
Less stubborn belly fat
Its best-known benefit. In large human trials, tesamorelin cut deep abdominal (visceral) fat compared with placebo in adults with HIV-associated lipodystrophy.
-
Less liver fat
Human trials found lower liver fat on tesamorelin, including in adults with fatty liver disease.
-
Memory and thinking with age
In a 20-week human trial in older adults, tesamorelin improved scores on cognitive tests compared with placebo.
-
Body composition as you age
It raises your own growth hormone and IGF-1, which fall with age. Lean mass in healthy adults has not yet been studied at scale.
-
Recovery and sleep
Often paired with other growth hormone peptides for recovery and deeper sleep, uses that have not been tested in trials yet.
- 412adults in its first phase 3 trial
- 15.2%less visceral fat on tesamorelin, against a 5.0% rise on placebo
- 2010the year the FDA approved Egrifta, the same active ingredient
- 2phase 3 trials behind that approval
How does Tesamorelin work?
Tesamorelin binds the same receptor as the body's own growth hormone releasing hormone. The pituitary responds by releasing its own growth hormone, in its natural pulses, which then raises IGF-1, the hormone that carries most of growth hormone's effects on tissue. Because the pituitary stays in charge of the release, the body's normal feedback keeps working. That is the practical difference between tesamorelin and injected growth hormone.
What does the research on Tesamorelin show?
- Human study
In a phase 3 randomized trial of 412 adults with HIV-associated lipodystrophy, visceral fat fell 15.2% on tesamorelin against a 5.0% rise on placebo. Falutz 2007, NEJM
- Human study
A second phase 3 trial confirmed the reduction in visceral fat. Falutz 2010
- Human study
A randomized trial published in JAMA found reductions in both visceral fat and liver fat. Stanley 2014, JAMA
- Human study
In adults with HIV and fatty liver disease, tesamorelin reduced liver fat compared with placebo. Stanley 2019, Lancet HIV
- Human study
In older adults, including some with mild cognitive impairment, a 20-week trial reported favorable effects on cognitive tests. Baker 2012
Tesamorelin side effects and safety.
- Joint pain
- Swelling in the hands and feet
- Muscle aches and tingling
- Redness or irritation at the injection site
- A rise in blood sugar in the early weeks, which is why fasting glucose is checked
These are the common effects on the approved product's label. Tesamorelin is not used in pregnancy, with an active cancer, or where the pituitary itself is the problem, and people with a history of certain tumors or diabetic eye disease need particular care. IGF-1 and fasting glucose are monitored while you use it. The World Anti-Doping Agency bans it at all times.
Before your first vial: baseline labs, IGF-1, a complete blood count (CBC), and a comprehensive metabolic panel (CMP), so every later result has a starting point.

Common questions.
What are the benefits of tesamorelin?
In large human trials, tesamorelin reduced deep abdominal fat and liver fat in adults with HIV-associated lipodystrophy, and a smaller trial in older adults reported favorable effects on cognitive tests. People also use it for body composition, recovery, and sleep, which have not been tested in trials. Its FDA approval covers the HIV population, so other uses are off-label, a decision your provider makes with you.
What are the side effects of tesamorelin?
On the approved product's label, the common effects are joint pain, swelling in the hands and feet, muscle aches, tingling, and injection-site reactions. Blood sugar can rise in the early weeks, so fasting glucose and IGF-1 are checked while you use it.
Is tesamorelin the same as Egrifta?
It is the same active ingredient. Egrifta is the brand-name product the FDA approved in 2010; the take-home vial prescribed here is not the Egrifta brand.
How much does tesamorelin cost?
A take-home vial is $200, and it includes the bacteriostatic water and syringes. The consultation is free, and any visit or labs your plan needs are quoted before you start.
Tesamorelin vs sermorelin: what is the difference?
Both ask the pituitary to release its own growth hormone. Sermorelin is the first 29 amino acids of the natural signal and acts briefly; tesamorelin is a modified version that lasts longer and has the larger, more recent human trial record. Which fits is decided at your consultation.
Is tesamorelin used for weight loss?
It is approved to reduce deep abdominal fat in adults with HIV-associated lipodystrophy, which is different from general weight loss. If losing weight is the main goal, the studied route is a GLP-1 medication in our medical weight loss program.
Is tesamorelin the same as growth hormone?
No. Growth hormone is the hormone itself. Tesamorelin is a signal that asks the pituitary to release its own growth hormone, in the pulses the body normally uses, so the body's own feedback stays in the loop.
What is monitored while taking tesamorelin?
Blood sugar and IGF-1, the hormone growth hormone produces downstream. The approved-product trials saw a rise in fasting glucose in the early weeks, so it is checked rather than assumed.
