What is SS-31?
SS-31, known generically as elamipretide, is a four amino acid peptide designed to reach the inner membrane of mitochondria, where cells make most of their energy. It is the same active ingredient as Forzinity, which the FDA approved in September 2025 to improve muscle strength in Barth syndrome, a rare inherited mitochondrial disease. Here it is prescribed as a take-home vial you inject under the skin, after a free consultation and a review of your history.
SS-31 benefits and uses.
-
Mitochondrial energy
A human study in older adults measured muscle energy production after a dose of SS-31.
-
Muscle strength
Its FDA approval rests on human trials in Barth syndrome, where muscle strength was the measure.
-
Healthy aging
Small, short human studies in older adults have looked at muscle energy, the basis for its use in the longevity conversation.
-
Paired with MOTS-c
Often discussed alongside MOTS-c as a mitochondrial pairing, a combination not studied in trials.
- 4amino acids, built to reach mitochondria
- 2025the year FDA approved Forzinity, the same active ingredient
- Every vialcomes with its own certificate of analysis
How does SS-31 work?
Mitochondria produce most of the cell's energy across an inner membrane whose structure depends on a fat called cardiolipin. Elamipretide binds cardiolipin and appears to stabilize that membrane and the energy-producing machinery embedded in it. In Barth syndrome, where cardiolipin is malformed, that is the basis of its approval; in healthy tissue the same binding is proposed to support energy production under stress.
What does the research on SS-31 show?
- Human study
FDA granted accelerated approval in September 2025 to improve muscle strength in Barth syndrome, the first treatment specific to that disorder. Shirley 2026
- Human study
In a 12-person crossover trial in Barth syndrome with an open-label extension, elamipretide was studied for strength and function. TAZPOWER
- Human study
A 168-week open-label extension followed people with Barth syndrome on elamipretide. 168-week extension
- Human study
In older adults, a single dose was studied for its effect on muscle energy production. Roshanravan 2021
SS-31 side effects and safety.
- Injection-site reactions are the main finding on the approved product's label.
- Its safety record comes from a few dozen people with a rare disease; in healthy adults it has been studied only in small, short trials.

Common questions.
What is SS-31?
SS-31, or elamipretide, is a small peptide designed to reach the inner membrane of mitochondria. It is the same active ingredient as Forzinity, which the FDA approved in 2025.
What does SS-31 do?
It binds cardiolipin in the mitochondrial membrane and appears to stabilize the machinery that makes cellular energy. In Barth syndrome it improved muscle strength enough for FDA approval.
Is SS-31 safe?
On the approved product's label, injection-site reactions are the main finding. That record comes from a few dozen people with a rare disease, and studies in healthy adults have been small and short.
Is SS-31 FDA approved?
Its active ingredient is, as Forzinity, approved in September 2025 for Barth syndrome. The take-home vial prescribed here is not the Forzinity brand.
How is SS-31 taken?
It is a small injection under the skin. Your dose and schedule are set by Catherine Seidle, APRN at your visit.
How much does SS-31 cost?
A take-home vial is $200, and it includes the bacteriostatic water and syringes. The consultation is free.
SS-31 vs MOTS-c: what is the difference?
Both are mitochondrial peptides. SS-31 binds the mitochondrial membrane and has an FDA-approved product behind it; MOTS-c is a signal encoded in mitochondrial DNA, studied so far in mice and cells.
