Does this sound familiar?

Signs it may be your hormones.

For most women the shift starts in the 40s, during perimenopause, often years before the last period. Receptors for estrogen and testosterone sit in every system of your body, so losing these hormones affects far more than hot flashes.

  • Hot flashes or night sweats
  • Brain fog, or losing words mid-sentence
  • Trouble falling or staying asleep
  • Anxiety, irritability or low mood
  • Weight that settles around your middle
  • Low sex drive or vaginal dryness
  • Tired even after a full night's sleep
  • Joint aches, or losing muscle and strength
  • Periods that are heavier, lighter or unpredictable
  • Thinning hair or drier skin

None of these prove a hormone problem on their own, which is why we test before we treat. Wondering whether it is perimenopause? Our perimenopause and menopause guide walks through the stages.

Estradiol, testosterone and progesterone, together.

We replace them with bioidentical hormones, optimized together, and use one lab marker, FSH, as the guide.

Estrogen

Estradiol

Your most potent estrogen. It protects the heart, bones and brain, especially when it is started early, and supports mood, memory and sexual health. We replace it with the same molecule your ovaries made.

Androgen

Testosterone

The most abundant active sex hormone in a woman's body. It supports energy, mood, libido, muscle and bone. At doses that keep levels in a healthy female range, masculinizing changes are uncommon.

If you have a uterus

Progesterone

Estrogen on its own can thicken the uterine lining, and progesterone protects it. We prescribe oral bioidentical progesterone.

The target

FSH below 20

FSH works like a thermostat. When estradiol is low, FSH rises, and when estradiol is adequate, FSH falls. After your first pellet we aim to cut your FSH in half, then bring it below 20 over time. In perimenopause FSH can swing from month to month, so we always read it with your symptoms and your other labs.

  • Goal zone: FSH below 20
  • Symptoms more likely: FSH above 20
  • Over time
  • After first pellet, about half
  • Before
FSH in mIU/mL. An example path; your own starting point and pace will differ.
How it is given

Why pellets are our first choice.

The same hormone works differently depending on how it is given. Pellets skip the liver, deliver estradiol and testosterone at doses set for you, and keep levels steady between visits.

FDA status. Compounded hormone pellets are not FDA-approved products. They are prescribed by licensed providers and prepared by licensed compounding pharmacies, with baseline labs, follow-up labs and dose adjustment behind every round. FDA-approved forms of these hormones are available too, and we offer creams, patches and injections when one of them fits you better.

Women's pellet therapy is $375 a procedure, the same price every round. Labs are separate. HSA and FSA accepted.

From the published studies

What estradiol and testosterone protect.

Findings from studies of groups of women. Timing matters, and your own picture depends on your history and your labs.

Heart 44%

Slower thickening of the artery walls when estradiol was started within six years of menopause, in the ELITE trial.

Bones 5.7%

More spine bone density in one year when women switched from oral estrogen to estradiol and testosterone implants.

Living longer 18 years

Of Women's Health Initiative follow-up, with no increase in deaths from any cause, heart disease or cancer.

Breast 11 vs 18

Breast cancers diagnosed over ten years among 1,267 women on testosterone pellets, against the 18 expected for their age.

The brain is part of this too. Women face a higher risk of Alzheimer's disease, and brain changes begin during the menopause transition, years before symptoms. And if you are working on your weight, testosterone helps you keep muscle while you lose, which is why hormone labs are part of our medical weight loss program for women 40 and over.

Is hormone therapy safe for women?

Most fear of estrogen traces back to one study, the 2002 Women's Health Initiative. When its first results made the news, many women stopped treatment or never started it.

That study tested a horse-derived estrogen with a synthetic progestin, in women who were 63 on average and often more than a decade past menopause. It did not test estradiol. When researchers followed those women for 18 years, they found no increase in deaths from any cause, heart disease or cancer.

Today the evidence supports hormone therapy as safe and effective for most healthy women with symptoms who start before 60, or within ten years of menopause, and menopause guidelines say it does not need to stop just because of age. Route matters too: oral estrogen passes through the liver first, which raises clot risk.

None of that makes it risk-free. Some histories change the answer, and we go through yours, with labs before treatment and after it. The full safety conversation is on our hormone replacement therapy page.

Common questions.

How much does hormone therapy for women cost?

Pellet therapy, our first choice, is $375 a procedure, the same price every round, and most women need a round every three to five months. Labs are billed separately, and the Full Panel is $175. Creams, patches and injections are quoted with your plan. We are cash-pay, HSA and FSA eligible, and financing is available through Affirm.

What does hormone therapy do for women?

It replaces the estradiol, testosterone and progesterone your body makes less of in perimenopause and after menopause. Women usually come to it for hot flashes, sleep, mood, energy and libido, and the longer-term aim is protecting the heart, bones and brain. We dose to your labs and your symptoms, with FSH below 20 as the guide.

Am I too old for hormone therapy?

Timing matters, and earlier is better: the evidence is strongest for women who start before 60 or within ten years of menopause. Menopause guidelines also say hormone therapy does not need to stop just because of age, so if you are already on it, age alone is not a reason to come off. If you are past that window and have not started, your history decides, and that is what your evaluation is for.

Do women need testosterone?

Yes. Testosterone is the most abundant active sex hormone in a woman's body, and it supports energy, mood, libido, muscle and bone. At doses that keep levels in a healthy female range, masculinizing changes are uncommon. Some women notice mild acne or extra hair growth, which usually settles with a dose adjustment.

Is hormone therapy safe for women?

For most healthy women with symptoms who start before 60, or within ten years of menopause, current evidence supports hormone therapy as safe and effective. Most of the fear traces back to the 2002 Women's Health Initiative, which tested a horse-derived estrogen with a synthetic progestin in women who were 63 on average, and its 18-year follow-up found no increase in deaths from any cause, heart disease or cancer. Your own history still matters, and we go through it with you before anything is prescribed.

Do I need progesterone?

If you still have a uterus, yes. Estrogen on its own can thicken the uterine lining, and progesterone protects it. We prescribe oral bioidentical progesterone.

What does FSH below 20 mean?

FSH works like a thermostat. When estradiol is low, FSH rises, and when estradiol is adequate, FSH falls. After menopause it is the marker we use to tell whether your body is getting enough estrogen. After your first pellet we aim to cut your FSH in half, then bring it below 20 over time. In perimenopause FSH can swing from month to month, so we always read it with your symptoms and your other labs.

How long do pellets last for women?

Most women need their next insertion every three to five months, about four on average. Your symptoms and your labs set the timing, and you call to schedule when the symptoms your pellets relieved start to come back.

Can hormone therapy help with weight?

It can help with the part hormones play. As estradiol falls, the body tends to store more fat around the middle, and testosterone helps you keep muscle while you lose weight, which matters on a GLP-1 medication. That is why hormone labs are part of our weight loss program for women 40 and over. Hormone therapy is not a weight loss drug, and the two programs can work together.

How do I start?

Book the free consultation, ten to fifteen minutes by phone or video. If hormone testing makes sense, labs come next, and you review them with Catherine Seidle, APRN before anything is prescribed. If pellets fit, placement takes a few minutes in the office.
